Functional Omics Core Laboratory (FOCL)

This form is used exclusively for the submission of projects that will be supported by the NIAID 75N93025C00014 FOCL Contract.

Please navigate to the Sample Submission/Submit your Project page for the intake of all other Genomics Core projects.

FOCL Project Submission

In addition to the Project Intake Form, we request that you complete the FOCL Project Submission Form below. This formally enters your project into the EPC Genomics Core Laboratory Management System and allows us to capture additional metadata.

Please use this form to submit your FOCL Project once the following are in place:

• Project Intake approved by NIH COR/CO
• Project Intake confirmed by the EPC Genomics Core
• Project ID assigned by the EPC Genomics Core
• Project Sample Manifest finalized

The FOCL Project Sample Manifest template is provided by the EPC Genomics Core after confirmation of Project Intake and needs to be filled out with as much information as possible before being attached to this submission.

Drop-off or shipment of samples can be arranged once your project has been recorded in our system.
